Advisor Notification of Student Exit Level Completion

This form is to be used by the advisor when a student completes an exit level, but the student does not want the printed award nor does the student want to participate in commencement.


Instructions: Complete all items below. Attached a completed and signed candidate plan of studies (CPS) to the form. Submit the form to the Academic Dean (Director or Department Head if there is no Dean) of the program of study for final approval. This form should be submitted at the beginning of February for fall completers, at the beginning of June for spring completers, and at the beginning of September for summer completers.
